Rise AMG: Vendor Registration Form

Please read carefully and review the enclosed documents. Before you may perform services on behalf of a client under management the Association requires the following documentation and information:

1. An executed Master Services Agreement (this web form)
2. Complete Vendor W-9 information (information to be filled out inside this form)
3. A Certificate of Insurance showing:
   1. Minimum of $1m in Liability Coverage
      1. RISE must be listed as an additional insured as shown in the example;
   2. Current Worker’s Compensation coverage
   3. Rise Association Management Group must be listed as a certificate holder.
